Survivorship Program
The Cancer Care Centers at Regions Hospital and HealthPartners Riverside Clinic established a Survivorship Program in 2008 to address the emotional and physical needs of patients as they finish their cancer treatment. The program consists of the Survivorship Clinic, educational and support services and the Cancer Survivor Advisory Council. Survivorship Program staff partner closely with patients to develop a cancer treatment summary and survivorship care plan that the patient and primary care physician can implement together.

Survivorship Clinic
In the Survivorship Clinic an interdisciplinary team reviews the patients treatment experience and the patients current physical and emotional well being.

Based on this review, the patient receives a comprehensive and individualized cancer treatment summary and survivorship care plan. Patients also receive information that identifies specialists and resources within HealthPartners and the community to manage specific survivorship issues including nutritional education, physical activity recommendations and other topics of interest to cancer survivors.

Survivorship Program Education and Support Services
  • Renewing Life, an 8-week transformative program of education and support for people living with cancer
  • Let's Get Physical, From Surviving to Thriving, a 6 week class giving survivors the tools and strategies for living a physically active life style
  • The Wellness Within support group for individuals living with leukemia, lymphoma and myeloma
  • Lung Cancer support group for patients and families
  • Taking Charge support group for women with breast cancer
  • Lundborg Library with DVDs, books, nutritional information brochures, physical activity strategies, and relaxation activities
Cancer Survivor Advisory Council
The Survivorship Program includes a Cancer Survivor Advisory Council that meets 6 to 8 times a year. The mission of the council is to advise us on issues that cancer patients face after treatment and to provide feedback and recommendations to improve the program.

Funding for the development of the Cancer Survivorship Program was made available by generous donations to the Regions Hospital Foundation's 2007 Wine Auction.
