  | Health Care Directives
Health care directives are documents that state your choices if you are unable to communicate your health care treatment preferences. They allow you to designate someone else to make decisions on your behalf as well as specify certain directions for your treatment. Please let your nurse know if you have completed a health care directive.
You can complete a health care directive while you are in the hospital. You may contact the Social Service office at (651) 254-9260 or the Patient Representative office at (651) 254-2372 for information or assistance.
