Frequently after hospitalization, seniors have additional health care needs. During that time, transitional care may be just what you need.
Our highly trained staff can continue to care for you in a transitional care facility. A physician and nurse practitioner, who specialize in senior health, lead a team dedicated to helping you regain your strength so you can return home.
Our staff will coordinate all your health care needs during your stay. We also work with the staff at the transitional care center to coordinate additional health care needs, such as rehabilitative care and discharge planning.
We involve you and your family in developing a plan of care and educate you about medications and other treatments, so you can continue on the path to health after you leave. We also update your personal physician and clinic about your health status to ensure a smooth transition when you are discharged.
