PartneringCare® provides comprehensive medical care at many area long term care facilities. We have physicians and nurse practitioners who are specially trained and interested in the care of these patients. Getting to know you and caring for you is our priority.
Our medical team provides health assessment, tests and treatments at the long term care facility, so you don't have to leave. We also coordinate specialty care, such as a visit to an orthopedic surgeon or rheumatologist. If you require urgent care, we offer easy access to care with a 24-hour call system.
If your medical needs become more complex and you require hospitalization, we will continue to coordinate your care so you can concentrate on what's important - getting better.
We work with you and your family to create a care plan and we consult with you about your preferences for care and any changes to the plan.
