Home Care
There may be times throughout your treatment when you are more comfortable and relaxed in your home. Our staff can help coordinate the transition into your home and work with you to determine your home care needs. Home care is available to support you as needed at any stage of illness.
Integrated Home Care
provides home care for patients and families in a caring, home-oriented program. We also work with many other Twin Cities home care services.

Hospice Care
Hospice is a program of care for people who are facing a terminal illness. Hospice care provides support for you and your family to help you remain in your place of residence as long as possible. Respite care is offered at Regions Hospital.
Hospice of the Lakes
provides full hospice care for patients and families. We also work with many other Twin Cities hospice care services.
