Home Care and Hospice

Home Care
Integrated Home Care offers a comprehensive range of home care services. The staff provide personal, professional and cost-effective care to meet patients' unique needs.

Services include:
  • Skilled nursing
  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Medical social work
  • Infusion therapy
  • Home health aide
  • Palliative care
  • Chaplaincy services
Suggested indicators for home care referrals:
  • Lives alone
  • Lacks adequate support systems
  • Recent diagnosis of illness or health condition requiring significant lifestyle adjustment
  • New diagnosis
  • Not accepting diagnosis
  • Unable to competently, safely or comfortably perform required treatments or other procedures
  • Needs continued rehabilitation
  • Needs to modify home or use adaptive equipment
  • Faces developmental or situational crisis
  • Frequent medication changes or complex medication management
  • Uses emergency center for primary care or less than 30-day readmit to the hospital
  • Risk of hospitalization

Hospice Services
Hospice of the Lakes provides a comprehensive end-of-life care program for patients and their families. With an emphasis on comfort and quality, hospice care provides support to help patients remain in their home setting.

Hospice care is provided by an interdisciplinary team of nurses, physicians, social workers, home health aides, volunteers and counselors.

Care can also be provided wherever patients live, including nursing homes, alternative residences or private homes. For patients admitted to Regions Hospital, we are proud to partner with the hospital's Palliative Care Team.

  • Grief counseling and bereavement support
