Privileges

Credentialing is a term that encompasses two separate processes: credentialing and privileging.

Credentialing

Primary source verification of healthcare providers education, training, work experience, licensure, and other related information provided by the applicant.

Privileging

Granting approval for an individual to perform a specific procedure or scope of practice based on documented competence in the specialty in which privileges are requested.

The Medical Staff application process is divided into four steps. Please following the directions in each step carefully:

  1. Review the following documents:
    Promises
    Vendor relations
    Paging protocol
    Bylaws
    Governing document: credentialing policy
    Governing document: fair hearing policy
    Governing document: investigations and corrective action
    Governing document: quality improvement and peer review
    Governing document: standards of professional conduct
  2. Complete Delineation of Privileges (DOP) form.
  3. Complete online credentialing application ApplySmart and Regions Hospital specific documents.
  4. Submit application fee.
