Our Survivorship Program addresses the emotional and physical needs of patients as they complete their cancer treatment. The program consists of an individualized cancer treatment summary, a personal visit with a specialist in survivorship issues, psychological and mental health support and community resource referrals.
Survivorship Clinic
In the Survivorship Clinic, an interdisciplinary team reviews the patient’s treatment and current physical and emotional well being. Based on this review, the patient receives an individualized survivorship care plan.
The plan identifies specialists and resources within HealthPartners and the community to manage issues such as nutritional education and physical activity recommendations.
Cancer Survivor Advisory Council
The Survivorship Program includes a Cancer Survivor Advisory Council which meets six to eight times a year. The mission of the council is to advise our Cancer Care Center on issues that cancer patients face after treatment and to provide feedback and recommendations for improving the program.
